For your convenience, and to facilitate a more efficient registration, you may print the forms below and bring the completed forms to the Clinic for your first visit.
Each patient will need each of the following forms with the exception of the Accident Form, which you will need only if you are visiting our offices as a result of an accident.
Please bring the following:
- Insurance cards
- Photo ID
- Films and/or records pertaining to your visit
